Provide professional and compassionate social work services to patients and their families within the Yale New Haven Health System.
Conduct comprehensive biopsychosocial assessments to identify patients’ needs and develop individualized treatment plans.
Collaborate with interdisciplinary teams to ensure holistic and coordinated care for patients.
Offer emotional support and counseling to patients and their families to help cope with illness, treatment, and other challenges.
Connect patients and families with community resources and services to address social, economic, and environmental factors impacting their health.
Advocate for patients’ rights and work towards eliminating barriers to their care.
Monitor and evaluate patients’ progress and adjust treatment plans accordingly.
Document patient interactions and maintain accurate and confidential records in accordance with professional and legal standards.
Participate in continuing education and training to stay current on best practices and developments in the field of social work.
Maintain a positive and collaborative working relationship with colleagues and other healthcare professionals.
Adhere to ethical standards and guidelines set forth by the National Association of Social Workers (NASW) and the Yale New Haven Health System.
Promote a culture of diversity, inclusivity, and cultural competence within the organization.
Act as a resource for colleagues and provide guidance and support in complex cases.
Participate in quality improvement initiatives and contribute to the development of policies and procedures related to social work services.

Job Qualifications
Master’s Degree In Social Work From An Accredited University.
Active Licensure As A Clinical Social Worker (Lcsw) In The State Of Connecticut.
Minimum Of 3-5 Years Of Experience Working In A Clinical Setting, Preferably In A Healthcare Or Hospital Setting.
Knowledge Of Evidence-Based Treatment Modalities And Experience Conducting Individual And Group Therapy Sessions.
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Provide Culturally Sensitive Care To Diverse Patient Populations.
