Coordinate Care: The Nurse Navigator will be responsible for coordinating the care of patients, working closely with healthcare providers to ensure seamless and effective treatment plans.
Advocate for Patients: The Nurse Navigator will serve as a patient advocate, advocating for their needs and ensuring they receive the best possible care and support.
Provide Support: The Nurse Navigator will provide support to patients and their families throughout their healthcare journey, offering guidance, resources, and emotional support.
Educate Patients: The Nurse Navigator will educate patients about their condition, treatment options, and resources available to them, empowering them to make informed decisions about their healthcare.
Collaborate with Healthcare Team: The Nurse Navigator will collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure a comprehensive and coordinated approach to patient care.
Assess Patient Needs: The Nurse Navigator will assess the needs of patients, including physical, emotional, and financial needs, and connect them with appropriate resources and support.
Monitor Patient Progress: The Nurse Navigator will monitor the progress of patients throughout their treatment, identifying any barriers or challenges and working to address them.
Maintain Documentation: The Nurse Navigator will maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress, in compliance with legal and ethical standards.
Stay Current with Healthcare Trends: The Nurse Navigator will stay informed about current healthcare trends, advancements, and resources, and incorporate them into patient care as appropriate.

Job Qualifications
Active And Unrestricted Rn License In The State Of Florida.
Bachelor’s Degree In Nursing (Bsn) Or Higher From An Accredited Institution.
Minimum Of 3-5 Years Of Clinical Experience In An Acute Care Setting.
Certification In Case Management Or Patient Navigation Preferred.
