Provide compassionate and skilled nursing care to patients in their homes.
Conduct thorough assessments of patient’s health status and develop individualized care plans.
Administer medications and treatments as prescribed by physicians.
Monitor and document patient’s progress and report any changes to the healthcare team.
Educate patients and their families on disease management, medication administration, and other healthcare topics.
Collaborate with physicians and other healthcare professionals to ensure coordinated and effective care.
Maintain accurate and up-to-date patient records.
Adhere to all healthcare regulations and standards.
Ensure patient safety and comfort at all times.
Communicate effectively with patients, families, and healthcare team members.
Participate in ongoing education and training to maintain and improve skills and knowledge.
Advocate for patients and their needs.
Maintain confidentiality and professionalism at all times.
