Advocate for community members by addressing social determinants of health and connecting them to resources and services.<\/p>\n
Develop and implement community-based health programs and events.<\/p>\n
Collect and maintain accurate data on community health needs, outcomes, and progress.<\/p>\n
Assist with program evaluation and identify areas for improvement.<\/p>\n
Maintain a thorough understanding of local, state, and federal health initiatives and policies.<\/p>\n
Serve as a liaison between the community and healthcare providers to improve access to healthcare services.<\/p>\n
Participate in regular training and professional development opportunities to enhance skills and knowledge.<\/p>\n
Demonstrate cultural competency and sensitivity in all interactions with community members.<\/p>\n
Represent the ROOTS program and the City of Pittsburgh in a professional and positive manner.<\/p>\n
