Develop and maintain relationships with law enforcement agencies, social service providers, and community organizations to support the goals of the LEAD program.<\/p>\n
Conduct assessments and develop individualized treatment plans for clients experiencing substance abuse and mental health issues.<\/p>\n
Collaborate with clients to identify and connect them with appropriate resources and services, such as counseling, housing, and job training.<\/p>\n
Advocate for clients’ needs and rights within the criminal justice system and other institutions.<\/p>\n
Monitor clients’ progress and provide ongoing support and guidance to assist them in achieving their goals.<\/p>\n
Maintain accurate and confidential client records, documentation, and reports.<\/p>\n
Participate in regular meetings and trainings to stay informed about best practices and updates in the field of substance abuse and mental health.<\/p>\n
Serve as a liaison between clients and their families, as well as other community support systems.<\/p>\n
Work closely with law enforcement to facilitate diversion of individuals with substance abuse and mental health issues from the criminal justice system into treatment and support services.<\/p>\n
Continuously evaluate and assess the effectiveness of the program and make recommendations for improvements.<\/p>\n

Job Qualifications<\/p>\n
Bachelor’s Degree In Social Work, Criminal Justice, Or Related Field.<\/p>\n
<\/p>\n
Minimum Of 2 Years Of Experience Working In A Case Management Role, Preferably In A Criminal Justice Setting.<\/p>\n
<\/p>\n
Knowledge Of And Experience With Harm Reduction Principles And Evidence-Based Practices In Criminal Justice Interventions.<\/p>\n
<\/p>\n
Strong Communication And Interpersonal Skills, With The Ability To Engage And Build Rapport With Diverse Populations.<\/p>\n
<\/p>\n
Understanding Of The Lead Model And Its Implementation In A Law Enforcement Setting, As Well As Knowledge Of Local Resources And Services For Individuals Experiencing Homelessness Or Substance Use Disorders.<\/p>\n
