Develop and implement personalized nutrition plans for patients and residents according to their individual needs and health goals.<\/p>\n
Conduct thorough assessments of patients’ nutritional status and make recommendations for dietary changes as needed.<\/p>\n
Provide ongoing education and counseling to patients and residents on proper nutrition and healthy lifestyle habits.<\/p>\n
Collaborate with interdisciplinary teams to ensure coordinated and holistic care for patients and residents.<\/p>\n
Maintain accurate and up-to-date records of patients’ nutrition plans, progress, and any necessary adjustments.<\/p>\n
Stay current on industry trends and research in the field of nutrition to continuously improve and enhance services.<\/p>\n
Monitor and track patients’ adherence to prescribed nutrition plans and make adjustments as needed.<\/p>\n
Advocate for and promote a culture of health and wellness within the facility.<\/p>\n
Communicate and work closely with food service staff to ensure meals meet patients’ dietary needs and preferences.<\/p>\n
Participate in facility meetings and trainings to provide nutrition expertise and support to other team members.<\/p>\n
Adhere to all state and federal regulations, as well as company policies and procedures, in the provision of nutrition services.<\/p>\n
Demonstrate a commitment to providing compassionate and person-centered care to patients and residents.<\/p>\n
Maintain confidentiality and professionalism in all interactions with patients, residents, and team members.<\/p>\n
Collaborate with community resources and organizations to provide nutrition education and outreach programs.<\/p>\n
Continuously assess and evaluate the effectiveness of nutrition programs and make recommendations for improvements as needed.<\/p>\n

Job Qualifications<\/p>\n
Bachelor’s Degree In Nutrition, Dietetics, Or A Related Field.<\/p>\n
<\/p>\n
Registered Dietitian (Rd) Certification From The Commission On Dietetic Registration (Cdr).<\/p>\n
<\/p>\n
Minimum Of 2 Years Of Experience In A Healthcare Setting, Preferably In A Long-Term Care Or Skilled Nursing Facility.<\/p>\n
<\/p>\n
Knowledge Of Federal And State Regulations Related To Nutrition And Dietary Services In Healthcare.<\/p>\n
<\/p>\n
Strong Communication And Interpersonal Skills To Effectively Educate And Counsel Patients And Collaborate With Interdisciplinary Healthcare Team Members.<\/p>\n
