Conduct thorough and accurate resident assessments to determine appropriate care plans and level of care needed.<\/p>\n
Collaborate with interdisciplinary team members to ensure all resident needs are met and documented.<\/p>\n
Maintain a comprehensive understanding of MDS and care planning processes to accurately complete resident assessments.<\/p>\n
Utilize clinical expertise to identify and prioritize resident needs and devise appropriate care plans.<\/p>\n
Ensure all assessments are completed in a timely manner and in accordance with state and federal regulations.<\/p>\n
Regularly review and update care plans to reflect any changes in resident conditions or needs.<\/p>\n
Oversee and assist with the implementation of care plans to ensure high-quality care is provided to residents.<\/p>\n
Act as a resource for staff and provide ongoing education and training on MDS and care planning processes.<\/p>\n
Monitor and analyze data related to resident assessments and care plans to identify areas for improvement and implement changes as needed.<\/p>\n
Maintain accurate and organized documentation for all resident assessments and care plans.<\/p>\n
Adhere to all privacy and confidentiality regulations when handling resident information.<\/p>\n

Experience In Long-Term Care: Pruitthealth Is A Long-Term Care Facility, So Experience In This Setting Is Crucial For A Case Mix Coordinator. Candidates Should Have A Minimum Of 2-3 Years Of Experience Working As An Lpn In A Long-Term Care Facility.<\/p>\n

Strong Understanding Of Mds And Rai Process: The Mds (Minimum Data Set) And Rai (Resident Assessment Instrument) Are Important Tools Used In Long-Term Care To Assess Residents’ Needs And Develop Care Plans.
