Coordinate and oversee the care of patients at PruittHealth, ensuring their needs are met and exceeded<\/p>\n
Act as the primary point of contact for patients and their families, providing support and guidance throughout their care journey<\/p>\n
Collaborate with interdisciplinary teams to develop and implement comprehensive care plans for each patient<\/p>\n
Conduct regular assessments of patients’ physical, emotional, and social needs to ensure the best possible care<\/p>\n
Communicate effectively with healthcare providers, patients, and families to ensure continuity of care and address any concerns or issues<\/p>\n
Manage and maintain accurate and up-to-date patient records and documentation<\/p>\n
Stay updated on current healthcare regulations and best practices to ensure compliance and provide quality care<\/p>\n
Advocate for patients and their families to ensure their needs are met and their rights are respected<\/p>\n
Provide emotional support and guidance to patients and their families during difficult times<\/p>\n
