Provide compassionate and skilled nursing care to patients in their homes.<\/p>\n
Conduct thorough assessments of patient’s health status and develop individualized care plans.<\/p>\n
Administer medications and treatments as prescribed by physicians.<\/p>\n
Monitor and document patient’s progress and report any changes to the healthcare team.<\/p>\n
Educate patients and their families on disease management, medication administration, and other healthcare topics.<\/p>\n
Collaborate with physicians and other healthcare professionals to ensure coordinated and effective care.<\/p>\n
Maintain accurate and up-to-date patient records.<\/p>\n
Adhere to all healthcare regulations and standards.<\/p>\n
Ensure patient safety and comfort at all times.<\/p>\n
Communicate effectively with patients, families, and healthcare team members.<\/p>\n
Participate in ongoing education and training to maintain and improve skills and knowledge.<\/p>\n
