Collaborate with interdisciplinary teams to develop and implement individualized care plans for patients.<\/p>\n
Conduct assessments to identify patients’ needs and determine appropriate resources for support.<\/p>\n
Connect patients with community resources such as housing, financial assistance, and mental health services.<\/p>\n
Advocate for patients’ rights and ensure their needs are being met.<\/p>\n
Monitor and evaluate the progress of patients’ care plans and make necessary adjustments.<\/p>\n
Provide crisis intervention and support for patients and their families.<\/p>\n
Maintain accurate and up-to-date documentation of all patient interactions and services provided.<\/p>\n

Job Qualifications<\/p>\n
