Coordinate Care: The Nurse Navigator will be responsible for coordinating the care of patients, working closely with healthcare providers to ensure seamless and effective treatment plans.<\/p>\n
Advocate for Patients: The Nurse Navigator will serve as a patient advocate, advocating for their needs and ensuring they receive the best possible care and support.<\/p>\n
Provide Support: The Nurse Navigator will provide support to patients and their families throughout their healthcare journey, offering guidance, resources, and emotional support.<\/p>\n
Educate Patients: The Nurse Navigator will educate patients about their condition, treatment options, and resources available to them, empowering them to make informed decisions about their healthcare.<\/p>\n
Collaborate with Healthcare Team: The Nurse Navigator will collaborate with other healthcare professionals, including physicians, nurses, and social workers, to ensure a comprehensive and coordinated approach to patient care.<\/p>\n
Assess Patient Needs: The Nurse Navigator will assess the needs of patients, including physical, emotional, and financial needs, and connect them with appropriate resources and support.<\/p>\n
Monitor Patient Progress: The Nurse Navigator will monitor the progress of patients throughout their treatment, identifying any barriers or challenges and working to address them.<\/p>\n
Maintain Documentation: The Nurse Navigator will maintain accurate and up-to-date documentation of patient interactions, treatment plans, and progress, in compliance with legal and ethical standards.<\/p>\n
Stay Current with Healthcare Trends: The Nurse Navigator will stay informed about current healthcare trends, advancements, and resources, and incorporate them into patient care as appropriate.<\/p>\n

Active And Unrestricted Rn License In The State Of Florida.<\/p>\n

Bachelor’s Degree In Nursing (Bsn) Or Higher From An Accredited Institution.<\/p>\n

Minimum Of 3-5 Years Of Clinical Experience In An Acute Care Setting.<\/p>\n
<\/p>\n
Certification In Case Management Or Patient Navigation Preferred.<\/p>\n
<\/p>\n
