Serve as the primary point of contact for patients and their families, providing support and guidance throughout their healthcare journey.<\/p>\n
Utilize clinical expertise to assess patients’ needs and develop individualized care plans.<\/p>\n
Collaborate with interdisciplinary healthcare team members to coordinate and facilitate seamless patient care.<\/p>\n
Educate patients and their families about their medical conditions, treatment options, and available resources.<\/p>\n
Advocate for patients’ needs and preferences, ensuring their voices are heard and their concerns are addressed.<\/p>\n
Coordinate and schedule appointments, tests, and procedures for patients, ensuring timely and efficient delivery of healthcare services.<\/p>\n
Monitor patients’ progress and provide ongoing support and education to promote self-management and adherence to treatment plans.<\/p>\n
Document patient interactions, assessments, and interventions accurately and in a timely manner.<\/p>\n
Participate in quality improvement initiatives and contribute to the development of best practices and standard protocols.<\/p>\n
Stay updated on current healthcare trends, regulations, and evidence-based practices to continually improve patient care and outcomes.<\/p>\n
Maintain confidentiality and adhere to ethical principles in all patient interactions.<\/p>\n

Job Qualifications<\/p>\n
Bachelor Of Science In Nursing (Bsn) Degree From An Accredited Institution.<\/p>\n

Current And Active Florida Registered Nurse (Rn) License.<\/p>\n
<\/p>\n
Certification In A Relevant Specialty Area, Such As Oncology Or Cardiology.<\/p>\n
