Provide exceptional care to patients with chronic or complex wounds.<\/p>\n
Work alongside experienced physicians and healthcare professionals to deliver comprehensive wound management services.<\/p>\n
Participate in the development and implementation of treatment plans for patients.<\/p>\n
Conduct thorough patient assessments and document findings accurately.<\/p>\n
Administer wound care treatments and procedures, including debridement, dressing changes, and wound irrigation.<\/p>\n
Educate patients and their families on wound management, prevention, and self-care techniques.<\/p>\n
Collaborate with other healthcare team members to ensure coordinated and effective care for patients.<\/p>\n
Communicate effectively with patients, families, and healthcare team members to provide updates, address concerns, and facilitate care coordination.<\/p>\n
Keep up-to-date with current wound care research, techniques, and protocols.<\/p>\n
Maintain accurate and timely documentation of patient care and treatment plans.<\/p>\n
Adhere to all infection control and safety protocols.<\/p>\n
Demonstrate a compassionate and empathetic approach towards patients and their families.<\/p>\n
Actively participate in team meetings and contribute to quality improvement initiatives.<\/p>\n
Maintain confidentiality of patient information at all times.<\/p>\n
Adhere to all policies and procedures of the APP Wound Clinic and Yale New Haven Health System.<\/p>\n
Maintain professional development and continuing education to enhance skills and knowledge related to wound care.<\/p>\n
Provide support and guidance to other staff members as needed.<\/p>\n

Job Qualifications<\/p>\n
Bachelor’s Degree In Nursing Or Related Field<\/p>\n
<\/p>\n
Valid And Current State Rn License<\/p>\n

Minimum Of 3 Years Of Experience In Wound Care Management<\/p>\n
<\/p>\n
Certification In Wound Care, Such As Wound Care Certified (Wcc) Or Certified Wound Specialist (Cws)<\/p>\n
<\/p>\n
Strong Clinical Assessment And Critical Thinking Skills, With The Ability To Develop And Implement Individualized Treatment Plans For Complex Wounds.<\/p>\n
