Provide compassionate care to older adult patients in the Acute Care for Older Adults unit.<\/p>\n
Utilize expertise and skills to assess, plan, implement, and evaluate patient care.<\/p>\n
Collaborate with interdisciplinary team members to develop and implement individualized care plans.<\/p>\n
Monitor and document patient progress and communicate any changes or concerns to the healthcare team.<\/p>\n
Administer medications and treatments as prescribed by physicians.<\/p>\n
Educate patients and their families on health conditions, treatments, and self-care techniques.<\/p>\n
Monitor and maintain patient safety and comfort.<\/p>\n
Maintain accurate and up-to-date patient records.<\/p>\n

Job Qualifications<\/p>\n
Bachelor’s Degree In Nursing: A Minimum Of A Bachelor’s Degree In Nursing From An Accredited Institution Is Required For This Position. Candidates With Higher Education Or Advanced Degrees Will Be Given Preference.<\/p>\n
<\/p>\n
Registered Nurse (Rn) License: Must Possess An Active And Unrestricted Rn License In The State Of Connecticut. Additional Certifications In Gerontology Or Acute Care For Older Adults Are Highly Desirable.<\/p>\n
<\/p>\n
Minimum Of Two Years Of Acute Care Experience: Candidates Must Have A Minimum Of Two Years Of Experience Working In An Acute Care Setting, Preferably With Older Adult Patients. This Experience Should Include Managing Complex Medical Conditions And Coordinating Care With A Multidisciplinary Team.<\/p>\n

Strong Clinical Skills: The Ideal Candidate Will Have A Solid Understanding Of Evidence-Based Practices And Guidelines For Caring For Older Adult Patients. They Should Have Experience With Assessing, Planning, Implementing, And Evaluating Patient Care Plans.<\/p>\n

Excellent Communication And Teamwork Abilities: The Rn In This Role Will Work Closely With Patients, Families, And Other Healthcare Professionals. Strong Communication And Teamwork Skills Are Essential For Providing High-Quality Care And Ensuring A Positive Patient Experience.<\/p>\n
