Provide professional and compassionate social work services to patients and their families within the Yale New Haven Health System.<\/p>\n
Conduct comprehensive biopsychosocial assessments to identify patients’ needs and develop individualized treatment plans.<\/p>\n
Collaborate with interdisciplinary teams to ensure holistic and coordinated care for patients.<\/p>\n
Offer emotional support and counseling to patients and their families to help cope with illness, treatment, and other challenges.<\/p>\n
Connect patients and families with community resources and services to address social, economic, and environmental factors impacting their health.<\/p>\n
Advocate for patients’ rights and work towards eliminating barriers to their care.<\/p>\n
Monitor and evaluate patients’ progress and adjust treatment plans accordingly.<\/p>\n
Document patient interactions and maintain accurate and confidential records in accordance with professional and legal standards.<\/p>\n
Participate in continuing education and training to stay current on best practices and developments in the field of social work.<\/p>\n
Maintain a positive and collaborative working relationship with colleagues and other healthcare professionals.<\/p>\n

Job Qualifications<\/p>\n
Master’s Degree In Social Work From An Accredited University.<\/p>\n
<\/p>\n
Active Licensure As A Clinical Social Worker (Lcsw) In The State Of Connecticut.<\/p>\n
<\/p>\n
Minimum Of 3-5 Years Of Experience Working In A Clinical Setting, Preferably In A Healthcare Or Hospital Setting.<\/p>\n
<\/p>\n
Knowledge Of Evidence-Based Treatment Modalities And Experience Conducting Individual And Group Therapy Sessions.<\/p>\n
<\/p>\n
Strong Communication And Interpersonal Skills, With The Ability To Collaborate With Interdisciplinary Teams And Provide Culturally Sensitive Care To Diverse Patient Populations.<\/p>\n
